Please DO NOT use the liability form from this page to add new players to your team.

PACKETS ARE NOT MAILED - FORMS MUST BE DOWNLOADED. This helps in reducing the cost for the league. We do appreciate your cooperation.

The following #1 - #4 forms are needed to make a complete packet. #5 - Individual Player Form - is not needed.

  1. COVER SHEET - (Roster-Contract) bring completed form to registration.

    To expedite the process make a copy for yourself and one to turn in at registration

    The Cover sheet will need to be completed .The manager’s or team's representative signature is required. The 3 part NCR will no longer be used.

    A receipt will be issued for all teams at registration if needed.

  2. ROSTER LIST FORM - include all players on your team. Please fill out completely. Incomplete information will delay your team’s registration. A minimum of 10 players is necessary for the roster. Make a copy of your roster for future reference.

    REMEMBER: W.C. residents will need to provide proof of residency. A copy of CDL, utility statement, credit card statement will do. These are returned to managers.

    If proof is not presented at registration a non-refundable non-resident fee ($10.00) will apply.

  3. LIABILITY FORM - Each player on your roster must sign a liability (waiver) and be included with your team packet. These forms may be faxed or e-mailed back and forth between players and managers.

  4. REQUEST FOR NIGHT OF PLAY - Three choices must be listed for teams. Teams may be placed on their second or third choice. On rare occasions a third may be offered, but placement is discussed with the manager before such a move is made.

    There is no guarantee of first choice, but we do try.

  5. INDIVIDUAL PLAYER FORM - If you are looking to get on a team, fill out the Find a Team. We DO NOT place players on teams, but do make the list available on our site for managers to view. Another way to join a team is to come to the fields during play, after 6:00 pm M-F (4:00pm Sundays) and talk to managers.

FEES ARE DUE at REGISTRATION - NO CREDIT CARDS ACCEPTED. CASH or CHECKS only. One check per team, made out to W.C.A.S.I. There is a $15.00 charge for returned checks.

Registration - 2017 SPRING/SUMMER team fees____$785.00___base fee.

Non-residency fees are $10.00 per player.

Field use fee assessment $10.00 per player on the roster, regardless of city of residence.

ALL OF THE FORMS MUST BE COMPLETE AND TURNED IN WITH PAYMENT AT REGISTRATION.

Refund policy: Full refund if canceled after registration and prior to teams posted. Partial refund (50%) if canceled after posting and before season schedule is posted. No refund after regular season schedule has been posted and season has begun.

REGISTRATION PACKETS DROPPED THROUGH THE SOFTBALL OFFICE MAIL SLOT WILL NOT BE ACCEPTED AND WILL BE RETURNED.

2017 SPRING/SUMMER ROSTER - CONTRACT

2017 - LIABILITY FORM - Needed for every player, coach & manager.

Added players (after registration) forms are available at the office. These are different than the regular liability form.

MONDAY, THURSDAY, FRIDAY - MENS (All Levels)

WEDNESDAY - MENS D

SUNDAY, TUESDAY, WEDNESDAY - MIXED (coed)

SUNDAY - OVER 40 MENS (Some teams during the weeknights may be designated OVER 40 and play in the Over 40 PLAY-OFFS.)
